Instructions To accompany Horse Indetity Diagram For all Passport Applications.

The instructions outlined on this form offers guidelines to vets and owners of how to correctly complete your equines identity diagram. It is important that diagrams are completed as outlined. If diagrams are incorrectly completed they will be returned.

Central Prefix Registration

Application for a Prefix can be applied for via the SSH in order that the Prefix is registered with the Central Prefix Register. The CPR will not accept direct applications this must be done through a Breed Society such as the SSH. Applications and Cheques should be sent and made payable to the SSH

Change Of Ownership

If a horse of pony changes hands please help us to keep our records up-to-date by completing the Change Of Ownership Form. This requires a signature from the previous and current owner. If the previous owners signature is not possible please contact the passport officer.Sale of horses, any person who buys a horse MUST within 30 days of purchase, return its passport to the Issuing body together with thier name & address or completed change of ownership form.

Failure to comply with this regulation is an offense and may be prosecuted.

Basic Passport Application Form

Persons wishing to register their horse or pony donkey or mule on a Basic Passport do not require to provide DNA samples as NO breeding will be recorded.As from the 1st July 2009, all new registrations, have to micro chipped, this must be carried out only by a Veterinary Surgeon, applications will only be processed, if completed by a vet.

Membership

All persons wishing to particpate at an SSH event as of 2008 must be fully paid members. Membership is due 1st of January annualy.

Payment not received on time will be charged the "New" membership rate.

Horse Pony overstamp Registration Form

This form is only to be used for horses or ponies that are already in possesion of an Equine Passport by persons wishing to Register their horse or pony with the SSH (overstamp)

All Horse / Pony mares and geldings MUST be "Registered" (Overstamped) by the SSH to enter an SSH Grading (Mare & Youngstock or Stallion Grading)

This form CANNOT be used as a BREED SOCIETY/ BASIC PASSPORT APPLICATION FORM

BREED SOCIETY PASSPORT FORMS Can ONLY be obtained directly from the Passport Officer as from 2007 ALLpassport applications MUST BE VERIFIED by DNA Testing. The passport officer will therefore post out Breed Society Passport Application Forms and DNA Kits to all applicants requiring them.

For owners wishing to enter a stallion to the SSH Stallion Grading please be aware that stallions must be FULLY PAID MEMBERS and Stallions registered with the SSH.

For Stallion Owners Wishing To Enter The Stallion Grading Please Download the following:

2) Stallion Grading Pack

3) Stallion X-Ray Protocol

4) Horse / Pony Registration Form (For Stallions Not Already Registered with the SSH

5) Membership Form (Membership is due annualy as of 1st January therefore Membership Form & Fees must be included by all stallion owners excluding life members)
